Break down complex doctor instrutions, treatment options into simple language. . Help families understand what to expect during treatment, recovery or terminal care. . Link patients to hospital packages, insurance details or outside community help after they leave the hospital. . Work closely with doctors, nurses and social workers to ensure the patient gets well rounded care. . Protect patient records and ensure all conversations remain confidential and safe.
